Why Your Baby’s Face Looks Different on 3D Ultrasound (And That’s Normal)

The Physics of the Womb

To understand why a 3D picture can look a little strange, we have to remember the environment. Your baby isn’t sitting in a photography studio with perfect lighting; they are floating in a dark pool of amniotic fluid, surrounded by a very snug muscular wall (the uterus), alongside a placenta and an umbilical cord!

Ultrasound machines use sound waves to map out the baby. (You can read a detailed explanation of how ultrasound works if you love the science!) When these sound waves hit the amniotic fluid, they travel cleanly. But if the baby is pressing their face against the uterine wall, the machine struggles to tell where the baby ends and the uterus begins.

The “Squished” Nose

This is the most common concern! If your baby has their face pressed up against the placenta or the wall of your uterus, their nose and lips will naturally look flattened out on the screen—just like if you pressed your nose against a glass window! Once they move back into the fluid, the nose “pops” back to its normal, adorable shape.

The “Puffy” Look

Sometimes the face looks a bit swollen or puffy. This happens when the sound waves have to travel through thicker maternal tissue or less-than-ideal amounts of amniotic fluid. The software does its best to stitch the echoes together, but it can occasionally result in a slightly “melted” appearance.

  1. Hydrate, Hydrate, Hydrate: The single best thing you can do is drink plenty of water in the weeks leading up to your scan. Amniotic fluid is the “window” the sound waves travel through. Clear, abundant fluid equals a clear, beautiful face.
  2. Timing is Everything: As mentioned in our complete ultrasound guide, the “sweet spot” is between 28 and 32 weeks. The baby is chubby enough to look like a newborn, but there is still room for them to move away from the uterine wall.
  3. A Little Sugar: If the baby is hiding behind their hands, drinking a small glass of cold fruit juice right before the session can sometimes get them moving!
